Consent to Collect Personal Data
I hereby give my consent to the Dental Practice (John G Plummer & Associates Dental Surgeons and / Faces & Smiles) to collect and process my personal data, including my name, contact details, and date of birth, as necessary for the provision of dental care services.
Consent to Collect and Use Health Data:I consent to the collection and use of my health and medical information, including my dental records (eg. clinical notes, models, imaging, x-rays, scans) medical history, treatment plans, and outcomes, for the purpose of providing me with dental care.
Consent to Contact:I agree that the Dental Practice may contact me via phone, email, or mail for appointments, treatment follow-up, and essential communications related to my dental care.
Data Sharing Consent:
I consent to the sharing of my personal and health data with other healthcare providers, laboratories, or specialists as necessary for my treatment and care.
Data Retention and Access:I understand that my data will be retained as required by law and that I have the right to access my personal data held by the practice upon request.
Revocation of Consent:I acknowledge that I have the right to withdraw my consent at any time by notifying our dental practice writing.
At our Dental Practices (John G Plummer and Associates Dental Surgeons and Faces & Smiles), we take photographs of our patients’ teeth and smiles for various purposes. This consent form outlines the terms under which these photographs may be used.
Confidentiality and Anonymity:
I understand that while my dental photographs may be used for the stated purposes below, my personal identity will not be disclosed without my further explicit consent, unless required by law.
Purpose of Photographs:
Dental photographs may be used for dental records, treatment planning, patient education, professional consultations, research, publications, and marketing related to the Dental Practices.
Scope of Use:
I understand that these photographs may be used in various formats and media including, but not limited to, digital, print, online, and social media platforms.
Withdrawal of Consent:
I acknowledge that I have the right to withdraw my consent at any time, which will not affect the standard of care I receive. However, I understand that it may not be possible to retract photographs already published or used in the public domain.
No Compensation
I understand that I will not receive financial compensation or other benefits in exchange for the use of my dental photographs.
Consent to Use Photographs:
I hereby give my consent to the Dental Practices (John G Plummer and Associates Dental Surgeons and Faces & Smiles) to take and use my dental photographs for the purposes outlined above.
As part of your dental assessment and treatment at our Dental Practices (John G Plummer and Associates Dental Surgeons and Faces & Smiles), it may be necessary to perform certain imaging procedures, including dental X-rays, Cone Beam Computed Tomography (CBCT) scans, and intra-oral imaging. This consent form outlines the use and importance of these procedures.

Purpose of Imaging Procedures:

Confidentiality and Data Protection:
I understand that any images taken will be used solely for the purpose of my dental assessment and treatment. Our Dental Practice assures me that only relevant information will be captured and stored in a confidential manner, adhering to data protection laws.
Risks and Benefits:
I acknowledge that while there is a minimal risk of radiation exposure from X-rays and CBCT scans, these imaging procedures are crucial for a thorough dental assessment and ensuring effective treatment. The benefits outweigh the risks, and the procedures will be conducted following all safety protocols.
Revocation of Consent:
I understand that I have the right to withdraw my consent at any time before the procedure.
Patient Declaration:
I confirm that I have read and understood this consent form. I have had the opportunity to ask questions and have them answered satisfactorily. I agree to the dental imaging procedures as described above for my dental assessment and treatment.
Consent to Proceed:
I hereby consent to undergo dental X-rays, CBCT scans, and intra-oral imaging as deemed necessary by my dental care providers at our Dental Practices (John G Plummer and Associates Dental Surgeons and Faces & Smiles)
John G Plummer & Associates is a training practice and within our School of Dental Nursing we train our own dental nurses, who work under supervision within our surgeries.
What is the dental nurse studying?
Our dental nurses are studying a City & Guilds Level 3 Dental Nursing qualification. This is a competency-based qualification that allows the trainee to work towards qualifying as a dental nurse and then register with the General Dental Council as a dental care professional within the scope of a dental nurse. It is a mandatory requirement for all dental care professionals to be registered with the General Dental Council in order to work within the UK.

What is the General Dental Council?
The General Dental Council (GDC) is the UK-wide statutory regulator of more than 120,000 members of the dental team. Their primary purpose is to protect patient safety and maintain public confidence in the dental profession. To achieve this, they are responsible for registering qualified dental professionals, setting standards for the dental team, investigating complaints about dental professionals’ fitness to practise, and working to ensure the quality of dental education.

Observation of trainee dental nurses within the surgery environment:
As part of the trainee dental nurses’ qualification, they are required to be observed in a realistic dental environment with patients. Observation is integral to the valid collation of assessment evidence and provides rich feedback on the trainee dental nurses’ level of performance and their level of competence in the role, as well as any developmental requirements needed to support the trainee on their journey to successfully achieve the qualification.
On occasions our in-house assessors may be present in the surgery to observe the trainee dental nurse and how they perform supporting your appointment. Assessors will always seek your consent to confirm if you are happy for them to observe the dental nurse during your appointment.
What can I do to support this process?
All trainee dental nurses are identifiable by their name badge and if an assessor is present in surgery to observe the trainee dental nurse, they will seek your verbal consent prior to treatment being commenced.
If you have any further questions, or do not wish to be treated in a surgery where a supervised student dental nurse is present, then please discuss this directly with the practice receptionist. Please note it is your decision and choice to provide consent and will not reflect your treatment in any way.

How to Make a Complaint
Your local practice

If it is not possible to raise your concern locally, please submit details of your complaint within 12 months of the incident or when you became aware of the issue.

Our Procedure
We aim to acknowledge complaints within 3 working days.
We aim to provide a full written response as soon as possible, usually within 21 working days.
If an investigation takes longer, we will keep you informed, explain the reasons for any delay, and confirm a revised timescale.
Making a complaint will not affect your ongoing care, and all concerns are handled confidentially.
We keep comprehensive records of all complaints. Patients will receive a written response, signed by the Responsible Person, explaining how the complaint was considered, the conclusions reached, and any remedial actions taken.
